Mass Intention Request

Complete this form to submit a request for a family member or friend to be remembered at Mass. 

Please note the following for Mass Intention Requests:
For Mass intention to be placed in the bulletin and read at Mass: Lead time for requests is 3 weeks. 
For Mass intention to be read at Mass only (and not placed in the bulletin): Lead time is at least 5 days prior to the requested Mass date.
A member of our office staff will contact you to confirm arrangements. For questions or if your request is not within the time frames listed above, please call the parish office at 847-885-7700, Ext 102. to check availability. A suggested donation of $10 is requested. 
Ways to give your donation: 
If paying by check, please make sure to put on the memo line: Mass Intention Donation
1. Send it in the mail to the parish office.
2. Drop it off at the parish office.
3. Drop it off in the basket at Mass.
